The decision fields: filled from the reason conversation
Reason given, in the employee's words, with the date it was given. Protected reason raised: yes or no, and if yes, which of FMLA leave, an accommodation or military service, and what documents were requested or received. Decision: the absence is excused and scores nothing; or the absence is not shown to be protected and the notice failure stands; or the absence is protected but notice could have been given under the usual procedure and the notice question is decided separately, as 29 CFR 825.303 contemplates. Decided by, and the date. These fields are what turn a form into a record of a decision, and a template without them produces a write-up that assumes what it should have asked. The reason field also tells a reviewer that the employer heard the employee before scoring anything.

The three fields usually left blank, and why they matter
The response box, which the employee may use and the employer keeps whether or not it agrees; left blank, the file shows the employee was never asked, which the EEOC's retaliation guidance and the National Labor Relations Board's protection of concerted activity both make a poor position. The prior warning field, stating the last step given, its date and its total; left blank, the write-up has lost its place in the sequence. And the protected-reason field above; left blank, the employer has assumed the answer to the one question that can void the write-up.
