A no call no show write up sample is what a small employer copies at ten at night, so this page is the text itself, block by block, written so it can be filled from the record and not from memory. It is followed by the faults that recur in the samples found online, because copying a bad sample produces a confident write-up that cannot be defended. The sample assumes the points policy used across this site, where a no-call no-show scores 2 points against a threshold of 8 in a rolling year, and a call-in rule that names a number and a notice period. The employee attendance policy guide on this site states that rule; the attendance points calculator supplies the total the sample cites.
Blocks one to three: the heading, the shift, the notice rule
Heading: Written warning, no-call no-show. Employee, role. Date of this notice. Issued by. Policy and version date. Shift: On Tuesday the twelfth you were scheduled to work the 6:00 am shift and did not work it. Notice: The attendance policy, paragraph 4, requires that a shift you cannot work be reported to the shift line at least two hours before it starts. No call, text or message was received before or during the shift. We attempted to reach you at 6:10 am and 6:30 am and left messages. Those three blocks establish the event, and they are written as facts with times. A sample that opens with a sentence about the importance of reliability has spent its first line on an opinion the employee will argue with.
Blocks four to six: the reason, the points, the total
Reason: On Wednesday you told us the absence was due to a family emergency. Having considered it, we have decided that the notice required by paragraph 4 could have been given and was not. If the absence was for a reason protected by law, please tell us and provide any supporting documents, and this notice will be reviewed. Points: Under paragraph 5 a no-call no-show scores 2 points. Total: With this event your record holds 5.5 points in the last twelve months against a threshold of 8. The events are listed below with their dates and points. The reason block is the one most samples omit, and it is the block that shows the employer asked, decided and left the door open, which 29 CFR 825.303 and the notice rules around it make necessary.
Blocks seven to nine: the next step, the response, the signatures
Next step: Under paragraph 6 a final warning is given at 6 points and employment ends at 8. Your oldest event will leave the twelve-month window on the date shown, when your total will fall to 5. Response: space for the employee to write, kept with this notice whether or not we agree. Signatures: employee, acknowledging receipt and not agreement; issuer; a line to note a refusal to sign with a witness. The event table sits under the total block: date, shift, event, excused or not, points. That is the entire sample, and it fits on one page because every block is a fact from the record. The employee can check every line against the roll call and the policy, and so can anyone reading the file later.
The faults in the samples found online
Four recur. Adjectives where numbers belong: unacceptable, serious, repeated, none of which the employee can check, in place of 2 points and 5.5 of 8. No notice rule: the sample says the employee failed to call without quoting the rule that says whom to call and by when, so the no-call cannot be distinguished from an absence. No total: the sample records the event and not where it leaves the employee, so the next step is a surprise. No reason line: the sample assumes the absence was unexcused, which is the assumption that produces a write-up an employer has to withdraw when the absence turns out to be protected leave or an accommodation. A fifth, less common, is a threat beyond the policy; the policy's own next step is already the strongest true sentence available. Records of the events themselves are kept in any case, and 29 CFR 516.2 sets out the hours records an employer must hold, which the attendance record should sit beside.
Questions people ask about no call no show write up sample
What should a no call no show write up sample say?
The shift by date and time; the call-in rule quoted by paragraph, with the attempts made to reach the employee; the reason given and the decision made, with an invitation to raise a protected reason; the points scored; the total against the threshold with the events listed; the next step; a response box; signatures.
Can I use a sample from a template site?
Only after checking it for the four faults: adjectives instead of numbers, no notice rule quoted, no total, and no reason line. Most fail at least two. The blocks on this page are written so each can be filled from the record.
Does the write-up need the employee's signature?
It needs the employee to have received it. The signature acknowledges receipt, not agreement; a refusal is noted with the date and a witness, and the write-up stands. The employee's written response is kept with it either way.
