A no call no show write up is the first piece of paper most small employers write about attendance, and the one most likely to be written from memory: the date is right, the rest is prose. A write-up that stands does four things, and none of them is prose. It names the shift that was missed, states the notice rule the employee did not meet, states the points the event scored under the policy, and states the total now in the window against the threshold. This page gives the no call no show write up example that structure produces, a sample worked on one employee, and the template a small employer fills from the record, with the free attendance policy template on this site stating the policy it cites, with no account.
The four things a no call no show write up has to cite
The shift: date, scheduled start, and that the employee did not work it. The notice rule: the policy's line stating who must be called, on what number, how long before the shift, and that no notice was received inside that window, which is what makes the event a no-call rather than an absence. The points: what a no-call scores under the policy in force that day, 2 on this site's worked example. The total: the points now in the rolling window, the threshold, and the room left. A write-up with those four is a record; a write-up that says the employee's failure to attend is unacceptable is a sentence. The policy paragraph each of the four rests on should be cited by number so the employee can read it.
Worked on one employee, line by line
Shift missed: Tuesday the 12th, scheduled 6:00 am, not worked. Notice: the policy requires a call to the shift line at least two hours before the start; no call, text or message was received before or during the shift. Points: under the policy dated the 1st of January a no-call no-show scores 2 points. Total: with this event the employee has 5.5 points in the rolling twelve months against a threshold of 8, 2.5 points of room; one further no-call plus one absence, or three absences, would reach the threshold. Next step: this is a written warning; the reminder at 6 points and the final warning at the threshold are stated in the policy. That is the whole sample, and every line of it came from the record and the five numbers.
What fills each line, and where it comes from
The template has the same four blocks with the employee's name, the shift, the notice rule quoted from the policy, the points and the total, then a line for the next step, the policy version cited, and signature lines for the lead and the employee with a line to note if the employee declines to sign. Each block is filled from the record: the shift from the roster, the notice from the roll call, the points from the policy, the total from the calculator. The free attendance points calculator on this site works the total and the room on the page, and RollCallWorks Pro drafts the write-up from the events it keeps, ready for the conversation and filed against the person with the policy it cited.
The no-call that must not be written up
Before the write-up is issued, the reason has to be asked, because a no-call that turns out to have been an emergency covered by protected leave under the FMLA, an absence that is a reasonable accommodation under the ADA or a service obligation under USERRA scores nothing and the write-up must not issue. The Department of Labor's and the EEOC's pages linked below describe each. The write-up template has a line for the reason given and the decision made, so that when the event was excused the record shows it at zero with the reason, and when it was not the write-up shows the question was asked.
Questions people ask about no call no show write up
What should a no call no show write up include?
The shift missed, the notice rule the employee did not meet, the points the event scored under the policy, the total now in the rolling window against the threshold with the room left, the next step, the policy version cited, and signature lines. Every line comes from the record and the policy's five numbers.
Is there a free no call no show write up template?
The structure on this page is the template: four blocks, a next step, the policy version and signatures. The free attendance policy template on this site states the policy it cites and the free points calculator works the total. RollCallWorks Pro drafts the write-up from the events it keeps.
How many no call no shows before a write-up?
The policy decides. Most small employers write up the first no-call because it scores the highest points (2 on this site's example against a threshold of 8) and the write-up is the record that the notice rule was explained. The reminder and final warning come at the totals the policy names.
